Summary:


Manages the clinical and day-to-day operations of the care coordination teams. Collaborates with leadership for care coordination across the continuum of care. Performs the care coordination duties of collaborating with the interdisciplinary medical teams in the provision of patient care, for performing a clinical review of selected patients to determine service needs, and for coordinating the care of selected patients within the organization. Integrates and coordinates access and utilization management, proactive patient management, care facilitation, and treatment planning functions. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.

Qualifications:

Required: Possess one of the following: • Bachelors of Science in Nursing, and 5 years of clinical experience and current Ohio RN licensure • Master's Degree in Social Work and current Ohio licensure as a Licensed Independent Social Worker - Supervisor (LISW-S). Five years of degree-related experience in a clinical setting, some of which includes staff and student supervision, academic experience, and demonstrated educational leadership. Strong working knowledge of Case Management and Care Coordination principles within a large system. Proven outcomes and knowledge in discharge planning, patient throughput and care across the continuum. Ability to work independently and as a member of a multidisciplinary team. Demonstrates excellent interpersonal, verbal/written communication, delegation and problem-solving skills. Demonstrates strong analytical, data management, and computer skills. Demonstrates strong leadership, organizational, time management, and personnel management skills. Preferred: MSN or Master's degree in health care related field. Two years of experience with case management, access management, home-health care, discharge planning, or related experience. Specialty certification in professional field. Knowledge of Milliman Care Guidelines (MCG) or similar industry product. Knowledge of EPIC functionality. Knowledge of case management software.
