Shift: 8AM-5PM

Summary:


Helps patients and family members navigate successfully through the health care system, by removing barriers to care, including providing access to resources, support and enhancing communication with the health care providers. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.

Qualifications:

Required: High School diploma or passage of a high school equivalency exam. Good computer skills including Microsoft Office software products. Demonstrated effective communication and interpersonal skills. Ability to work in a team environment. Culturally sensitive and comfortable working with a diverse urban population. Organized and detail oriented. Able to work a flexible schedule including some evenings or weekends. Reliable transportation to make patient home visits as needed. Staff assigned to Infectious Disease Current Ohio Driver's License along with a safe driving record that does not exceed 6 points. Preferred: Bachelors degree in a related field. 5 years of medical health care experience. Experience navigating health care for an individual (whether for themselves or another) with a diagnosis relative to the assigned area. Bilingual or multilingual skills. Physical Demands: May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. May need to remain still for extended periods, including sitting and standing. Ability to communicate in face-to-face, phone, email, and other communications. Ability to read job related documents. Ability to use computer.
