Summary:
** Ambulatory Internal Medicine Pharmacist position; collaborates with providers under consult agreement **
Provides direct patient medication-related care services to improve a patient's quality of life. Promotes pharmaceutical care and/or a specialty per the assigned clinical area (i.e., an opioid stewardship, critical care, etc.), which may include the ambulatory environment in any of The MetroHealth System's locations, or in an in-patient setting. Makes medication-related decisions on drug therapy regarding selection, dosages, routes, methods of administration, medication therapy monitoring, the provision of medication-related information, and counseling to individual patients in assigned clinical areas. Works with physicians and other healthcare professionals to improve the use of medications through formulary review, drug usage evaluations, quality monitoring, and improvement initiatives. Provides training/educational support for the development/enhancement of clinical skills in professional staff, pharmacy students, and pharmacy residents. Supervises technical staff in drug preparation as indicated. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.
Qualifications:
Required: Bachelor of Science degree in Pharmacy from a State of Ohio Board of Pharmacy approved institution.
Possess a current Pharmacist license from the State of Ohio Board of Pharmacy and maintains this license.
Meets one of the following:
• 3 years of experience as a Staff Pharmacist
• 1 year of experience working in a full-time pharmaceutical role with a specialty emphasis for a clinical practice or medical center.
• An advanced degree in Pharmacy with residency training.
Meets one of the following:
• Documented experience as a Practitioner in a Pharmaceutical stewardship, or a specialty area.
• Documented educational training and demonstrable knowledge in a pharmaceutical stewardship, pain management, and/or a specialty area.
May require on call, weekend or evening commitment depending on the service area business hours
Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds.
Preferred:
Advanced degree in Pharmacy as Doctor of Pharmacy from a State of Ohio Board of Pharmacy approved institution.
Clinically oriented Master of Science degree in Pharmacy practice and completion of Pharmacy Practice residency training.
Completion of an American Society of Health-System Pharmacists (ASHP) accredited residency. Current Board of Pharmaceutical Specialties (BPS) certified Pharmacist.
Physical Requirements: May sit, stand, stoop, bend, and ambulate intermittently during the day.
May need to sit or stand for extended periods. See in the normal visual range with or without correction.
Hear in the normal audio range with or without correction.
Finger dexterity to operate office equipment required.
May need to lift up to twenty-five (25) pounds on occasion.
Ability to communicate in face-to-face, phone, email, and other communications.
Ability to see computer monitor and departmental documents.
Requires some exposure to biohazardous chemicals.
