Summary:
In conjunction with the Nurse Manager, implements, coordinates, and evaluates all activities impacting Institute for H.O.P.E.™ Community Health Workers (CHW). Coaches and evaluates appropriate CHW-led interventions to ensure quality, cost-effective patient care and community impact. Works with the manager to support Community Health Worker operations, including departmental and financial management, and manages the clinical and day-to-day operations of the Institute for H.O.P.E.™ CHWs. Identifies and implements new models of care using CHWs to promote efficient and effective patient care across the continuum, including nurturing partnerships to reduce social needs and eliminate health disparities
Qualifications:
Required: Graduate of an accredited school of professional nursing or social work Bachelor of Science in Nursing or Master's in social work (MSW/MSSA) Currently registered nurse licensure in the State of Ohio or Current LISW-S in the State of Ohio At least 3 years' experience as a Social Worker or Nurse in a coordination capacity. Demonstrates strong interpersonal, verbal/written communication, delegation and problem-solving skills. Demonstrated ability to identify and effectively resolve problems/issues. Demonstrated ability to embrace and lead the team through change initiatives and to adapt to quickly evolving situations. Demonstrated strong critical thinking, abstract thinking, and analytical skills Ability to work effectively in a team environment. Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds. Preferred: Experience with or as Community Health Worker Specialty certification in the field. Experience in supervising/managing staff. Physical Demands: May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. May need to remain still for extended periods, including sitting and standing. Ability to communicate in face-to-face, phone, email, and other communications. Ability to read job-related documents. Ability to use a computer.
