Virtual Care Program Associate-Virtual Care Enterprise
Location: BROOKLYN HEIGHTS
Biweekly Hours: 80.00
Shift: 10 hour shifts with various start times between 7a-9a
The MetroHealth System is redefining health care by going beyond medical treatment to improve the foundations of community health and well-being: affordable housing, a cleaner environment, economic opportunity and access to fresh food, convenient transportation, legal help and other services. The system strives to become as good at preventing disease as it is at treating it. Founded in 1837, Cuyahoga County's safety-net health system operates four hospitals, four emergency departments and more than 20 health centers.
Summary:
Contributes to an optimal patient care experience by supporting the MetroHealth System's (MHS) vision of optimal patient outcomes, financial viability, innovation, and outstanding service. Advances the goals of the virtual care enterprise by facilitating patient participation and an optimal patient experience in virtual care initiatives. Supports innovative care delivery models (e.g., Hospital in the Home, Virtual Chronic Disease Management, etc.) that offer a subset of patients an alternative to traditional health care, including highly specialized in-home acute-care services. Participates in data collection, abstraction, analysis, and reporting. Ensures patients have necessary equipment and supplies to support their care provision virtually. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.
Qualifications:
Required: Associate degree in health care, technology, social services or a related field or any equivalent combination of education, training, and experience in addition to the experience stated below. Six months' relevant experience in healthcare setting. Available to work a variable schedule (some weekends and holidays may be required). Displays the ability to be collaborative and a quick study. Proficient in Microsoft Office Suite (Word, PowerPoint, Excel, Visio, etc.). Possesses a high degree of analytic ability. Demonstrated ability to manage multiple priorities. Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds. Preferred: Bachelor's Degree in a related field. Prior healthcare industry experience. Working knowledge of an electronic health record a plus. Physical Demands: Requires lifting of objects > 30 pounds and walking several miles. May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. May need to remain still for extended periods, including sitting and standing. Ability to communicate in face-to-face, phone, email, and other communications. Ability to read job related documents. Proficiency with computer systems and applications.
