Supervisor Discharge Planning Specialists - Care Management
Location: METROHEALTH MEDICAL CENTER
Biweekly Hours: 80.00
Shift: 8:30am - 5:00pm

Summary:
Supervises Discharge Planning Specialists and clerical staff, supporting discharge processes and communicating regarding safe/timely patient discharges. Provides direction and support for staff, promotes staff development, and collaborates with leadership to ensure the completion of goals and strategies. Collaborates with Social Workers and RN Case Managers on patient discharges. Manages precertification and escalation process for post-acute care placements. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.
Qualifications:
Qualifications: Required: Associate degree in health-related field, in lieu of a degree, five years of related experience. Three years of related experience. Strong quantitative skills, including demonstrated proficiency with Microsoft Office Suite and information systems. Excellent interpersonal skills and customer service skills. On-call, weekends, holiday rotation, and travel to other sites, as well as after-hours support. Excellent written, verbal, and interpersonal communication skills. Strong analytical and problem-solving skills. Strong critical-thinking skills. Ability to multi-task, prioritize work, and meet deadlines. Ability to work in a fast-paced environment. Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds. Preferred: Bachelor's Degree in Business, Health Administration, or related field. Supervisory experience. Progressive leadership experience. General understanding of health insurance payor processes and requirements Thorough understanding of Epic functionality. Strong working knowledge of systems such as OnBase, Lawson, and Kronos. Physical Demands: May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. May need to remain still for extended periods, including sitting and standing. Ability to lift at minimum 50lbs. Ability to communicate in face-to-face, phone, email, and other communications. Ability to read job related documents.
