The Heart Failure Care Center (HFCC) within The MetroHealth System is seeking an Advanced Practice Nurse/NP to join an excellent HF care team of providers (Board-certified HF cardiologists, NPs, clinical Pharmacist, HF nurse coordinator, HF-certified nurses, and social worker). The team at the HFCC provides care for a growing population of patients with various types and stages of HF, cardiomyopathy, and pulmonary hypertension, utilizing in-person clinic visits, telemedicine encounters, HF education sessions, and telemonitoring technology. Candidates are expected to collaborate with HF cardiologists and other team members to provide outpatient direct and indirect patient care, telemonitoring, and patient education, with prospect to future involvement in inpatient care. Candidates with expertise and/or interest in the management of adult patients with HF are encouraged to apply. One year of experience working as a provider is required.

Qualifications:

Required: MSN or grandfathered under Ohio Revised Code Graduate of an accredited advanced practice registered nurse program. Current registered nurse licensure in the state of Ohio Certificate of Authority as an Advanced Practice Nurse from the Ohio Board of Nursing Certification as Advanced Practice Registered Nurse in specialty. Current Basic Life Support (BLS) from the American Heart Association or obtains within 90 days of hire. Obtains other advanced life support certifications applicable to practice area within 180 days of hire. Meets the requirements/qualifications for clinical privileges in the specialty/department with The MetroHealth System and must report to Medical Staff Office at time of hire to apply for clinical privileges. Maintains a standard care arrangement and renews/signs it as required by the Ohio Board of Nursing. Preferred: Have or be able to apply for a Certificate to Prescribe/Externship from the Ohio Board of Nursing.
