Summary:


In conjunction with the HUB Operations Team and Better Health Partnership leadership, coordinates client referrals, outreach, and enrollment for the Better Health Pathways HUB to ensure a high-quality experience for clients of the HUB, including at-risk pregnant women, infants, children, and adults with chronic disease and/or behavioral health conditions. Builds trusting relationships with referral partners and the community health workers (CHWs) of the Care Coordination Agencies (CCA) that partner with the HUB. Works closely with the HUB Operations Team to identify opportunities for new referral partners in the community. Upholds the mission, vision, values, and customer services standards of The MetroHealth System.

Qualifications:

Required: Associates Degree, or in lieu of degree, any equivalent combination of education, training, and experience in addition to the experience stated below. Two years combined progressively responsible administrative support experience in a community or healthcare setting. Excellent interpersonal skills along with experience in working with multi-disciplinary groups. Demonstrated proficiency with Microsoft Office Suite. Excellent communication, and presentation skills. Detail oriented. Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds. Preferred: Four years combined progressively responsible progressively responsible administrative support experience in a community or healthcare setting. Community Health Worker certification by Ohio Board of Nursing. Physical Requirements: May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. Ability to use computer. May need to lift up to twenty-five (25) pounds on occasion. Ability to communicate in face-to-face, phone, email, and other communications and meetings.
